Important Forms
- The Mentor Background Check needs to be completed online. Mentors will need to create an account and then submit the form and their driver's license via the "Volunteer Application System." Please let them know that even though the system is called "Volunteer" that it is simply a way they provide the information necessary for a background check to keep our students safe; they are not expected or required to do anything else with or at the school in order to mentor you.
- Mentor Agreement Form to guide a discussion with your mentor on what your mutual expectations and goals are for the internship. This is a required form for your senior project in English class.
- Senior Project Proposal form which will be completed during the summer Google Classroom.
- Example Project Proposal
- Internship Details form: fill out so we know how to contact you for the summer.
- Hours Log with final mentor verification as well.
- Evaluation to be filled out by the mentor three times during the internship.
